Program Description
<br /> <br />
Prevention and Recovery in Early Psychosis (PREP) is a new program at FSA that operates in partnership with the University California, San Francisco (UCSF) and the Mental Health Association (MHA). This program is dedicated to early detection of psychosis and early delivery of care with a multi-disciplinary approach to treatment. The program combines a series of evidence-based treatments in Strength-based Care Management, Medication Management, Multi-family Group Therapy, Cognitive Behavioral Therapy for Early psychosis, Vocational rehabilitation and Cognitive Training to address the issue early and bring the symptoms to full remediation

Job Description
<br /> <br />
Under the direction of a licensed psychiatrist, exercises medical responsibilities for psychiatric evaluation, medication assessment and medication monitoring. Performs crisis intervention, supportive counseling and other direct mental health services, including case management, as needed. Assists with 5150 evaluations, as needed. Orders diagnostic tests, communicates medical issues with Primary Health Care provider. Maintains client caseload and clinical records in accordance with mandated standards. Directs treatment planning; and works as a member of multi-disciplinary team. Consults and provides on-going education to non-medical staff in psychopharmacology and medical aspects of patient care. Services will be provided either at FSA itself, in patients homes, or in other field locations as needed. Additional duties as needed. <br /> <br />
Program Specific Responsibilities

	Responsible for psychiatric evaluation, medication assessment and medication monitoring

	Order diagnostic tests as needed

	Coordinate care with PREP treatment team at FSA and UCSF

	Coordinate and counsels client and family on medical issues <br />
	Maintain client caseload, as designated by Associate Director
<br />
	Maintain clinical records in accordance with mandated quality assurance standards

	Attend all clinical meetings, staff meetings, supervision meetings and committee meetings when appointed
<br />
	Consult and provides on-going education to non-medical staff in psychopharmacology and medical aspects of patient care

	Possible outreach on behalf of PREP in the community

Minimum Qualifications
<br /> <br />
	A minimum of 3 years of Nursing experience, in community mental health a plus
<br />
	Has training and experience in psychopharmacology
<br />
	Verifiable completion of training in an accredited Nurse Practitioner Program <br />
	Current Nurse Practitioner license, and furnishing license from State of California
<br />
	Current furnishing number, or ability to obtain one within six months of employment
<br />
	Experience in working with geriatric population
<br />
	Demonstrated commitment to the welfare of individuals and families with mental/behavioral health issues
<br />
	Knowledge of principles, methods, and procedures for diagnosis, treatment, and rehabilitation of physical and mental dysfunctions, and for career counseling and guidance
<br />
	Knowledge of the information and techniques needed to diagnose and treat human injuries, diseases and psychosis. This includes symptoms, treatment alternatives, drug properties and interactions and preventative healthcare measures
<br />
	Knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation; psychological research methods; and the assessment and treatment of behavioral and affective disorders
<br />
	Complex problem solving; identifying complex problems and reviewing related information to develop and evaluate options and implement solutions
<br />
	Critical thinking skills; using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems
<br />
	Social perceptiveness skills; being aware of others' reactions and understanding why they react as they do. <br />

Edits and forwards results to attending physician for analysis and interpretation. This position reports clinically to the Medical Director area and administratively to the Director of Health and Provider Services. Under the general supervision of the Associate Medical Director - LTC, this position provides annual comprehensive risk assessments to designated HPSM members in the CareAdvantage (Medicare) program. This position is also responsible for ongoing medical care management for selected HPSM LTC patients to manage length of stay and prevent recurrent readmissions where possible.

This position is additionally responsible to work, under the guidance of the Medical Director area, with the HPSM LTC provider network, in particular the nursing directors of LTC units, to further the availability of information about advance directives, the POLST form and other end of life documents to assist HPSM members and their families in the LTC setting in becoming aware of this important information.
<br /> <br />
Requirements: Valid and unrestricted license as Registered Nurse in California. Advanced Practice Training leading to certification as a Nurse Practitioner is needed. Geriatric Nurse Practitioner preferred; Family Nurse Practitioner with extensive experience working with older patients is an acceptable alternative. Experience in caring for patients in the LTC setting as well as knowledge of Medi-Cal and Medicare programs are highly desirable. Knowledge of managed health care, the HMO industry, and the regulatory environment of managed care is preferred. Ability to work effectively in a multidisciplinary team setting is needed. Excellent clinical and analytical skills, ability to exercise good judgment in problem solving and decision-making situations, and excellent oral and written communication skills are needed.

Part-Time Physician Assistant or Nurse Practitioner to work in our Medical Clinics surrounding the Bay Area. The job involves commuting between one of our two clinics in San Francisco and San Leandro three days a week. We are looking for someone who does not mind commuting and is energetic, friendly, well trained, hard working team player who is self motivated and confident working independently. Days can be flexible throughout the position provided we are provided with a proper notice. Candidates will provide medical consultation, evaluation, and pharmacological services in an outpatient setting to patients with a full range of health issues.

For more than 30 years, the Marin Community Clinic, just north of San Francisco, has kept its doors open to low-income and uninsured individuals, providing them with the best in culturally responsive, primary healthcare. In fact, MCC is the largest such provider in the county, serving a population that stretches from Marin City in the south, through San Rafaels Canal District and north to Novato. <br /> <br />
MCC offers primary care services for both adults and children at our clinics in Greenbrae, San Rafael and Novato. Our clinics currently average over 50,000 patient visits annually. Patients are seen by a staff of committed family practitioners, internists, pediatricians, and nurse practitioners. <br /> <br />
POSITION SUMMARY:
<br />
The Nurse Practitioner handles primarily adult patients. The core philosophy is individualized care. The Nurse Practitioner focus is on patients conditions as well as the effects of illness on the lives of the patients and their families. The Nurse Practitioner makes prevention, wellness and patient education priorities. Informing patients about their health care and encouraging them to participate in decisions are central to the care provided by the NP. Collaborating with physicians and other health professionals as needed, including providing referrals. Counseling and educating patients on health behaviors, self-care skills, and treatment options. Obtaining medical histories and conducting physical examinations. Diagnosing, treating, and monitoring chronic diseases (e.g., diabetes, high blood pressure). Providing health maintenance care for adults, including annual physicals.

MINIMUM POSITION REQUIREMENTS:

Education &#38; Experience: <br />
The candidate must first complete the education and training necessary to be a registered nurse (RN). Must possess a CA Registered Nurse License. Nurse Practitioner Furnishing License. Successful completion of a Nurse Practitioner program that conforms to the standards of CA Board of Registered Nursing. Certification through the CA Board of Registered Nursing as a Nurse Practitioner. Three years of progressively responsible nursing experience. Two years of clinical experience as a Nurse Practitioner. Combination of education and experience that demonstrates the knowledge and abilities listed above will also be considered. <br />
<br />
Special Skills/Equipment:
<br />
Computer skills required. Current BLS certification. Bilingual (Spanish/English) skills are highly preferred. Excellent communication skills, both written and verbal. Sensitive to the needs and situations of a multi-cultural population from a variety of income levels.
