			<content:encoded><![CDATA[<p></p><p>The Wellness Center at the Family Medicine Residency of Idaho (FMRI), Idahos Ryan White Part C grant-funded HIV clinic system, seeks a nurse practitioner or physicians assistant to join our dynamic and growing clinic. This position is part of a clinical team that includes three HIV specialist physicians, an HIV physician fellow, as well as nursing and case worker staff. The ideal candidate will be experienced in managing primary medical care and, ideally, management of antiretroviral therapy for HIV positive patients. This position also provides services focused on patient education, HIV prevention, preventative health, and promoting wellness. The Wellness Center is part of a medical residency training program and all staff additionally participate in the education of nursing, mid-level, medical students and medical residents. This is an excellent opportunity to apply your clinical skills and experience to a diverse population of patients living in Southern Idaho and Eastern Oregon.</p>
<p>Clinical care competencies must include management of: asthma, hypertension, hyperlipdemia, diabetes, behavioral health, preventative health, minor surgical procedures, womens health and general primary care. </p>
<p>Clinical experience with infectious disease and/or HIV is highly desirable. </p>
<p>Mentoring and oversight by HIV specialists and ID physicians, as well as support for HIV-specific CME, is also available depending on candidate qualifications.</p>
<p>This position is 1.0 FTE and requires no overnight call or weekend duties. </p>

<p>Clinical Responsibilities: The Family Medicine Physician is accountable directly to the Medical Director. The position is responsible for providing primary medical care to patients, following approved protocols, maintains the required productivity numbers expected by Community of Hope, as set by the board, maintains consistent chart documentation, has an appropriate clinical knowledge base for specialty, is able to make independent clinical decisions with limited assistance from colleagues as required, provides care efficiently, cooperates with clinical and office policies, participates in quality improvement activities, does follow up on labs, and reviews results with patients in a timely manner, refills prescriptions requests in a timely manner, shares on-call responsibilities with other providers, participates on Quality Assurance Committee and in quality assurance activities, communicates with other medical providers on issues of patient care, is compliant with federal regulations, HIPPA, OSHA etc, is current with on-going training requirements and CMEs as required for licensing, and performs all other duties as assigned. </p>
<p>Qualifications Required: Medical Degree (MD/ DO, Board certified) with specialization in family practice and / or internal medicine required. Possess all needed certification including CLEA and DEA required. Continuous medical education and experience in infectious disease and HIV/AIDS, with a minimum two years experience practicing in a inner  city setting.</p>

Coordinate clinic operations to ensure provision of excellent patient care, outstanding customer service, and patient education. Partner with the Primary Care Clinic Manager to develop long and short-range strategic plans, administer and manage budget, and ensure operational efficiency. Act as a resource and role model for clinic staff. </p>
<p>Any combination of equivalent education and experience that provides the required knowledge and expertise is qualifying. Typical qualifications would be a registered nursing degree from an accredited school of nursing, current Minnesota license to practice as an RN, current BLS/CPR certification, and three years experience in clinic specialty with at least one (1) year in a supervisory capacity. Experience using a computerized health record preferred.</p>
<p>Excellent verbal, written and interpersonal communication skills. Excellent clinical nursing skills, and ability to deliver safe, competent patient care. Must be detail oriented and flexible with the ability to prioritize and creatively problem solve, displaying sound judgment and independent initiative. Must have the ability to lead and maintain a team-oriented, multi-focused atmosphere. Must be able to work independently with little supervision. Must have basic personal computer skills. </p>

Attends clinician meetings, monthly Center team meetings and project meetings as requested. Serves as committee liaison as needed.

REQUIREMENTS: <br />
	Satisfactory completion of an accredited training program. <br />
	Current license in the State of California as a Family Medicine physician, board certified or board eligible. <br />

Recent additions include a new Cardiac Cath Lab (opened in November 2006), and the installation of a new $1.6 million CT Scanner.<br /><br />As a Cardiology/Electrophysiology Nurse Practitioner, provides care including history and physical examinations, diagnostic tests, medical treatment and medical intervention to adult cardiac patients or those with risk factors ranging from 18 years old and up.&#160; Disease states covered, but are not limited to atrial and ventricular tachyarrhythmias, bradyarrhythmias, syncope, congestive heart failure, hypertension, valvular heart disease, chest pain/myocardial infarction, lipid management, peripheral vascular disease, DVT, as well as post-surgery/intervention management.&#160; <p>As a Cardiology/Electrophysiology Nurse Practitioner, provides care including history and physical examinations, diagnostic tests, medical treatment and medical intervention to adult cardiac patients or those with risk factors ranging from 18 years old and up.&nbsp; Disease states covered, but are not limited to atrial and ventricular tachyarrhythmias, bradyarrhythmias, syncope, congestive heart failure, hypertension, valvular heart disease, chest pain/myocardial infarction, lipid management, peripheral vascular disease, DVT, as well as post-surgery/intervention management.&nbsp; Functions independently seeking collaborative advise when necessary in the inpatient hospital setting and outpatient office setting.<br /> &nbsp;<br /> The EP advanced practitioner has primary contacts with patients/families, the EP Medical Director, Chief of Cardiology, Medical Director and Director of Cardiovascular Services, as well as referring physicians and their office staff, Nursing, Clinical Laboratory, Noninvasive Cardiology, Cardiac Catheterization Laboratory, Electrophysiology Laboratory, Medical Records, Business Services and other hospital departments and the community at large.</p>

BACKGROUND: The Indian Health Center is a nonprofit, community based organization that has been serving the American Indians, their families and other diverse cultural populations for more than twenty years. The Center offers Medicine, Dentistry, Community Health, substance abuse treatment, Counseling, a WIC program, a Medical Nutrition program, health education, outreach, and case management.
<br /> <br />
POSITION SUMMARY: Provides primary care in a Family Practice setting. Ambulatory services only, no E.R., in-patient, or on-call required.
<br /> <br />
DUTIES AND RESPONSIBILITIES:
<br />
	Provides primary care to adults, with an emphasis on healing maintenance and preventive care.
<br />
	Performs charting.
<br />
	Assists in conducting Quality Assurance reviews.
<br />
	Performs other duties that may be required to insure the effective operation of the clinic.
<br />
	Complies with the IHCs HIPPA and Privacy Policies.
<br />
	Assists in the IHCs chronic disease and health management programs.
<br />
	Provides family planning and womens health services- preferred.
<br />
	Provides care to 18 year olds and older.
<br />
	Plans, develops and implements a health education program for patients.
<br />
	Performs other duties as assigned.

	Provides Chronic pain management if needed

	Supervise FNP or Mid-level practitioners as needed

WORKING CONDITIONS:
<br /> Will be working in a fast paced non-profit community health clinic environment. Will need to be able to walk, stoop, bend, lift and carry up to 25 lbs. without aid. Will do extensive work on the computer and the telephone in communicating with staff, responding to inquiries, etc. Normal working hours will be from 8:00 am to 5:00 pm although hours may vary depending upon need. Will need to work independently and meet deadlines while also being an effective team member.

MINIMUM QUALIFICATIONS:

	Possession of Medical Doctor degree or D.O. degree.
<br />
	Board Certified or Board eligible in Internal Medicine.
<br />
	One year of clinical experience in a clinic environment.
<br />
	Possession of CA Medical License in good standing.
<br />
	Must be insurable for liability as a General Practitioner.
<br />
	Current knowledge and skills in adult primary care.
<br />
	Ability to maintain good working relationships with staff.
<br />
	A strong commitment to care for an underserved population.
<br />
	Experience in providing care for multi-cultural populations.
<br />
	Experience in working with diabetics and geriatric populations.

	Superlative customer service skills.

In this vital role, you will work with an extraordinary and dynamic team of physicians to provide direct, primary in-patient care to 10 to 14 seniors in the Medical Acute Care Unit (MACU) of Hebrew Rehabilitation Center, Boston Massachusetts. The MACU, licensed as an LTAC, is designed to accommodate the clinical needs of post surgical and post ICU medically complex patients. Reporting to the Physician-in-Chief, you will perform medical examinations, evaluations, assessments, diagnoses and treatment to MACU patients. You will also be relied upon to refer patients to specialists as appropriate. You will also be involved with directing and coordinating the patient care activities of nursing and support staff. Training and supervising medical students, residents and fellows engaged in specialty activities and procedures, is an important part of this role as well. <br /> <br /> Primary Responsibilities: This is a 40-hour full-time position (5 days a week) including occasional weekends. There are no overnights with this position. You must have internal medicine, geriatric, and/or physiatric experience in an acute care or long-term acute care facility. -Provide outpatient evaluation and treatment for injuries and illnesses.

-Perform physical examinations as needed to evaluate student's participation in sports, appropriateness for study abroad, birth control, or routine health screening.
<br />
-Provide preventative health care including birth control counseling, physical exams, pelvis exams, pap smears, breast and testicular exams.
<br />
-Present health education information and counseling with each visit to increase student's knowledge of illness, treatment and prevention.
<br />
-Provide travel counseling/medications/vaccinations for students/staff traveling internationally.
<br />
-Make referrals as needed to community resources and specialty medical care.
<br />
-Perform other duties as assigned.
<br /> <br />
KNOWLEDGE, SKILLS, AND ABILITIES:
<br />
-All skills necessary to provide high quality outpatient care to students.
<br />
-Good communication and organizational skills.
<br />
-Ability to handle and work with confidential information.
<br />
-Proficiency with Microsoft Word.
<br />
-Comfortable working at a remote site (no MD in the clinic).
<br /> <br />
REQUIRED QUALIFICATIONS:
<br />
1.Graduation from accredited nurse practitioner program.
<br />
2.Licensure and prescriptive authority in Washington State before beginning employment. <br />
3.Must have BLS certification before beginning employment.

4.Finalist candidates must satisfactorily complete pre-employment background check.

PREFERRED QUALIFICATIONS:

-Previous experience providing primary care.
