ORGANIZATION and MISSION

The East Bay Community Recovery Project (EBCRP) is a non-profit organization in Alameda County that aims to reduce the impact of substance abuse, mental illness, and incarceration in our community. We have offered a wide range of programs since 1989. We provide or coordinate comprehensive, high quality services that address the full spectrum of needs: treatment, education, employment, medical care and housing. We currently have two locations in Alameda County: Oakland and Hayward. EBCRP’s mission is to support the well being of the community by providing substance abuse, mental health and other health related services. We respect and encourage the strengths of individuals and families, and their ability to make healthier choices.

PROGRAM SUMMARY
The EBCRP Health Services program provides integrated substance abuse, mental health, HIV/AIDS and HEP C services to individuals and their families. Services include outreach, engagement, screening, assessment, treatment planning, individual, group and family counseling, psychoeducational groups, crisis intervention, peer support, limited medical services, and case management.

JOB SUMMARY

Under the direct supervision of the Division Director and the Medical Director, the Program Manager/Nurse Practitioner (NP) will manage and develop/expand holistic health services and will be part of a multidisciplinary team of licensed clinicians, certified substance abuse counselors and complementary care providers. The Program Manager will also minimally provide psychiatric services, such as: evaluation, medication evaluation and support, case consultation and documentation of services. The hours of the job will typically occur between the hours of 9AM – 5PM, Monday through Friday, typically 20 hours each week, and may require schedule flexibility.

ESSENTIAL JOB FUNCTIONS

• Oversee the development, implementation, and start up of the holist health services within the Health Program.

• Responsible for day-to-day program management.

• Provide supervision to a multidisciplinary staff.

• Attend meetings and all relevant training.

• Oversee and actively participate with the outreach efforts of the program.

• Responsible for some of the contract oversight and program outcomes.

• Other job duties as they develop in the upstart of this new program services.

• Minimally provide psychiatric services to clients with co-occurring substance abuse, mental health, HIV/AIDS and Hep-C in collaboration with a multidisciplinary team.
• Utilizes harm reduction philosophy & harm reduction strategies (i.e. motivational interviewing and stages of change)
• Maintain continuity of care for patients by making referrals to community agencies, and providing follow up for recently hospitalized and referred patients.

Documentation

• Ensure accurate and complete documentation per agency standards.

• Ensure accurate and complete data collection and reporting.

• Develop and document initial care plan and subsequent revised plans in the clients’ files in a timely manner.

• Maintain medical records, participate in quality assurance activities.

Collaboration

• Maintain close collaboration with the agency’s other program staff members, specifically regarding co- enrolled clients

• Participate actively in daily check-in meetings, weekly staff meetings, monthly case conferences and all agency staff meetings and trainings

• Ensure optimal client participation in care planning and service coordination

• In collaboration with the team have close involvement in program management and development of additional curriculum as needed

CERTIFICATION/QUALIFICATIONS
• Valid CA Nurse Practitioner license
• Valid California RN license.
• Minimum of one-year experience as a Psychiatric Nurse Practitioner.
• Current CA Driver’s license and clean DMV record.

• Use of an operable, insured, and registered vehicle during normal business hours to conduct agency business (mileage reimbursement for vehicle use is available)

• Experience and demonstrated understanding of substance abuse and mental health issues and their effects on incarcerated or formerly incarcerated individuals
• Management experience

• Experience and understanding of confidentiality laws and expectations.

• Well-organized and detail-oriented

• Excellent written, verbal and telephone skills

• Ability to handle a variety of tasks simultaneously with a problem-solving approach
• Demonstrated sensitivity to and experience working with persons from diverse ethnic, cultural, and economic backgrounds. Additionally, sensitivity to issues of sexual orientation, gender identification, homelessness, and individuals with disabilities including HIV/AIDS is required

• Desire and willingness to work well people with co-occurring mental health and substance abuse disorders complicated incarceration issues
• Knowledge of severe and persistent mental illness, substance abuse including treatment and medication, and the impact of illness on vocational functioning.

• Demonstrated ability to effectively and sensitively provide care to people from different cultural groups.

• Knowledge of current treatment (primary medical, substance abuse, and mental health) resources in Alameda County.

• Knowledge of Medi-Cal behavioral health services and documentation standards.

• Pre-employment drug screen required.

SALARY and BENEFITS
Salary of $46,800.00. EBCRP offers prorated health insurance and dental insurance.
