We are a private practice (west chicago). We are seeking a physician assistant/nurse practicioner to be part of our clinical team. Position wll be heavily involved with coordinating patient care in the nursing homes.
RESPONSIBILITIES: performing hospital rounds and conducting follow-up visits in the nursing homes
