The Health Plan of San Mateo (HPSM), a managed care health plan for Medi-Cal, Healthy Families, Healthy Kids, CareAdvantage, San Mateo ACE, and HealthWorx programs, seeks a Nurse Practitioner Long Term Care (LTC). This position reports clinically to the Medical Director area and administratively to the Director of Health and Provider Services. Under the general supervision of the Associate Medical Director – LTC, this position provides annual comprehensive risk assessments to designated HPSM members in the CareAdvantage (Medicare) program. This position is also responsible for ongoing medical care management for selected HPSM LTC patients to manage length of stay and prevent recurrent readmissions where possible.
This position is additionally responsible to work, under the guidance of the Medical Director area, with the HPSM LTC provider network, in particular the nursing directors of LTC units, to further the availability of information about advance directives, the POLST form and other end of life documents to assist HPSM members and their families in the LTC setting in becoming aware of this important information.
Requirements: Valid and unrestricted license as Registered Nurse in California. Advanced Practice Training leading to certification as a Nurse Practitioner is needed. Geriatric Nurse Practitioner preferred; Family Nurse Practitioner with extensive experience working with older patients is an acceptable alternative. Experience in caring for patients in the LTC setting as well as knowledge of Medi-Cal and Medicare programs are highly desirable. Knowledge of managed health care, the HMO industry, and the regulatory environment of managed care is preferred. Ability to work effectively in a multidisciplinary team setting is needed. Excellent clinical and analytical skills, ability to exercise good judgment in problem solving and decision-making situations, and excellent oral and written communication skills are needed.
