Job Title: Adult Medicine Nurse Practitioner
Location: West County at 141 & 40
Position available: Full time Monday – Friday
Requirements:
Experienced Nurse Practitioner in Adult Medicine
Current active and unrestricted Nurse Practitioner certificate for Missouri
Strong communication and interpersonal skills
High ethical standards
Ability to work well with all members of the patient care team
Self-motivated and directed
Strong interest in disease management within a large patient population
Able to learn and adapt to EMR tool.
Duties:
Work collaboratively with Physicians.
Maintain and develop Educational Programs for a large Diabetic population
Primary responsibility will be management of patient chronic medical problems in a primary care Internal Medicine outpatient setting.
Management of patients in hospital/nursing home setting
